 Fifth Grade Supply List for 2016-2017

3 composition notebooks (no spiral notebooks, no binders please)** Labeled with student name
1 pack of wide ruled notebook paper (hand in to teacher)
4 two pocket folders - Labeled with student name
1 two pocket folder with brads - Labeled with student name
6 packs of #2 pencils (hand in to teacher)
2 packs of pencil top erasers
1 large glue stick (keep in pencil pouch)
1 pack of coloring utensils (crayons or colored pencils) to keep in pencil pouch
1 pair of scissors to keep in pencil pouch
1 pack of Post - It - notes (hand in to reading teacher)
1 pencil pouch (cloth/canvas, no pencil boxes please) - Labeled with student name
3 boxes of Kleenex
1 pair of earbuds

1 container of hand sanitizer
1 box of sandwich zip-lock-bags

**Supplies will need to be replaced as needed throughout the school year.

Any parent wanting to attend field trips or volunteer with any classes in Davidson County, must have a completed  background check on file. Forms can be obtained through the school’s office or on the school’s website.

Get your code from the office (parent must show ID to get code and instruction packet). This is a great way to stay on top of your child’s academic performance and keep track of any absences.

Being Sick:
As per school board guidelines, please do not send your child to school sick. Children should be free from fever, vomiting or having diarrhea for 24 hours before returning to school. Please see pages 2 and 4 of the student handbook for more information about illness and absences.

 Positive, Accountable, & Working toward Success!!

Students have reviewed and discussed Wallburg’s rules and expectations (as outlined in our school “PAWS” Behavior and in the student handbook under conduct code). Appropriate, safe, and non-disruptive behavior is expected at all times and in all settings of our school and during school sponsored activities. Not following behavior expectations inhibits teaching and learning and can lead to unsafe conditions.

Each day a binder travels with each class and the teacher in each class initials any warnings or reminders needed throughout the day. One warning or reminder is allowed each day without a consequence (a verbal warning is given in each class by each teacher prior to writing any warnings in the book). Additional warnings or reminders needed for inappropriate, unsafe, or disruptive behaviors will cause consequences to occur [such as lack of social time during lunch (silent lunch), play time (walking laps during playtime: half of playtime for first consequence and whole playtime for second consequence; students walking laps will not be permitted to walk with friends and will walk at their own pace in various playground and recess areas being monitored by teachers), time out in another setting, parent contact, and office referral].

Students who have exemplary behavior (less than 3 reminders in the book over the month
period) will be given opportunities to obtain monthly rewards through our school’s Positive
Behavior Intervention and Support (PBIS). Some rewards may be extra recess time with grade level friends, Movie and Popcorn, MadScience, Minute-to-Win-It games, Football and cheerleading clinic, Winston-Salem Dash, Magician, Legos from HPU, etc.
